Appointment Information

To make an appointment please call our main number (512) 450-1300, Press 1
  • Office Hours – Monday to Friday: 8:00am to 5:00pm
  • Insurance – Please have your insurance card available so that we can help you obtain the optimum benefit. We are contracted with most managed care plans.
  • Appointment Time – We always strive to provide you with a convenient appointment time. If you require same day or next day care we will do our best to accommodate you while honoring our professional responsibilities to all patients
  • NEW PATIENT FORMS REQUIRED – Please print out the required new patient forms, complete them in advance and bring them with you.
  • Arrival – Plan to arrive 20 minutes ahead of your scheduled appointment if your new patient forms have not been completed in advance.
  • Referrals – If your insurance requires a referral from your primary care physician, contact that office and request the referral. Some offices may take 24-48 hours to approve and send your referral to us.

When You Come For Your Appointment

Please bring the following:
  • Driver’s License or a valid ID
  • Insurance information
  • Referral Letter (if required)
  • Reports, X-rays, MRI’s, CT scans and any other relevant information. If you have x-rays, MRIs or other imaging studies related to your injury, please bring them to your appointment. This will allow us to avoid duplicating the service and related cost
  • List of medications (if any)

CANCELLATION – If you need to cancel always call in advance

To avoid a missed appointment fee, please telephone the office during business hours at least 24 hours in advance so that we can offer your appointment time to patients on our waiting list. Likewise call in advance to reschedule and we will work with you to find the most convenient time for you.

ON TIME

We recognize that your time is valuable and we make every effort to run on time. However, your medical needs may require extra time, as may others. Our priority is the highest quality medical care for our patients and occasionally emergencies or patients whose care requires extra time can cause scheduling delays beyond our control.

PAYMENT

We will collect co-payments, as required by insurance plans, at the time of your appointment. We accept cash, checks and all major credit cards. Advance payments are requested for some services.
